Where 1 is the name of the file (without brackets and without the File: prefix), 2 is the name of the user who created it, and 3 (optional) is the label to display above the file.
The template is known to mess up the layout of images; when used with illustrated articles, it is suggested to put this template under Etymology sections where images are not used.
